Initial Consultation Agreement
Parties
This agreement is between you and Meridian Concierge Health L.L.C. (“Meridian”), a Wyoming limited liability company with its principal office at 11175 Cicero Drive, Alpharetta, Georgia 30022. It covers the private consultation you reserve by paying the $325 consultation fee.
Who conducts the consultation
Your consultation is conducted by a Meridian concierge. A licensed physician with the treating practice (currently ICCA) reviews your intake and records once you sign a Medical Records Authorization allowing Meridian to share them with the practice.
Physician review of your intake and records does not create a physician-patient relationship. That relationship begins only after a direct evaluation by a licensed physician.
What the consultation includes
A private conversation with your concierge about your goals and history; physician review of the intake and records you authorize Meridian to share; and a coordination summary of possible next steps, which may include a physician-directed evaluation.
The consultation does not include a diagnosis, a treatment plan, a prescription, or any procedure, and it does not determine whether you are a candidate for any therapy.
No specific outcome is promised or guaranteed. Services and any therapies are evaluated individually, offered only when clinically appropriate, and individual results vary.
Fee and credit toward your total
The consultation fee is $325, paid in advance. Payments are processed by Stripe; Meridian does not store card or bank details.
If you proceed after your consultation, the full $325 is credited toward the total in your Concierge Services Agreement and Financial Schedule. The credit is part of the total you pay, not an additional charge.
The fee is for the consultation and coordination described here. It is not payment for treatment and does not establish candidacy.
Scheduling
After your payment is received, your concierge contacts you to arrange the consultation at a mutually convenient time, or sends you a private scheduling link.
Rescheduling
You may reschedule at no charge by giving at least 24 hours’ notice before the scheduled time. Reply to your concierge or email ops@meridianconcierge.health.
Rescheduling with less than 24 hours’ notice, or after a missed consultation, is at Meridian’s discretion.
Cancellation and refunds
The $325 fee is non-refundable once paid, including if you cancel or decide not to proceed.
If Meridian cancels your consultation and cannot offer you a new time, Meridian refunds the fee in full.
Privacy
Your information is handled as described in the Privacy and Data Use Notice at https://meridianconcierge.health/privacy. Records are shared with the treating practice only under a Medical Records Authorization you sign.
General
Using this website or submitting an inquiry does not create a physician-patient relationship. That relationship is established only after a direct evaluation by a licensed physician.
This agreement is governed by the laws of the State of Georgia, except where a mandatory consumer-protection law of your residence applies.

Electronic acceptance
By accepting electronically, you agree that your electronic acceptance is your signature and that this document may be provided to you electronically. You can print or save a copy now, and you can ask ops@meridianconcierge.health for a paper copy at any time.
