These terms govern use of meridianconcierge.health and related Meridian Concierge Health coordination services. By using the site or submitting an inquiry you agree to these terms and to the privacy policy at https://meridianconcierge.health/privacy.

Information provided by Meridian Concierge Health is for educational purposes and does not constitute medical advice, diagnosis or treatment. Eligibility for any service or therapy requires an individualized evaluation by an appropriately licensed healthcare professional. Certain regenerative therapies may be investigational or may not be approved by the U.S. Food and Drug Administration for a particular use. Individual results vary.

Certain regenerative therapies — including some cell- and exosome-based products — may be investigational and may not be approved by the U.S. Food and Drug Administration for a particular use. State law does not change federal requirements. Any therapy is considered only when clinically appropriate, where legally available, and under the direction of a licensed physician.

No specific outcome is promised or guaranteed. Services and any therapies are evaluated individually, offered only when clinically appropriate, and individual results vary.

Using this website or submitting an inquiry does not create a physician-patient relationship. That relationship is established only after a direct evaluation by a licensed physician.

Access is by application and subject to medical evaluation. Not every applicant will be a candidate, and services are offered only when clinically appropriate and where legally available.

Submitting an inquiry, paying a coordination fee, or using the private portal does not establish a physician-patient relationship, does not determine candidacy, and does not entitle you to any therapy. Clinical services, if any, are determined only by a licensed physician after individualized evaluation, when clinically appropriate and where legally available. Individual results vary.

Fees are for private concierge, coordination, and evaluation-scheduling services. They are not payment for, and do not entitle the payer to, any medical treatment or therapy. Clinical services, if any, are determined and provided only by a licensed physician following individualized evaluation.

The $325 consultation fee covers a private consultation with your Meridian concierge, with physician review of the intake and records you authorize us to share. It is not payment for treatment and does not establish candidacy. If you proceed after your consultation, the full $325 is credited toward the total in your Concierge Services Agreement — it is part of the total you pay, not an additional charge. The fee is non-refundable once paid (unless Meridian cancels and cannot offer a new time), and you may reschedule at no charge with at least 24 hours’ notice.

Meridian Concierge Health may send optional coordination text messages to a mobile number you provide — for example scheduling, document requests, and appointment reminders. Message frequency varies. Message and data rates may apply. Texts are coordination only and will not include diagnoses, lab results, therapy names, or other clinical content. Consent is not required to request a consultation. You agree not to misuse the site, submit information you are not authorized to share, attempt to access another person’s records, or use the site to send unlawful or harassing content. We may refuse or close an inquiry when needed to protect privacy, security, or legal compliance.

The site is provided as-is for educational and coordination purposes. We do not warrant uninterrupted access or that content is complete for your circumstances. Information on this site is not medical advice.

To the extent permitted by law, Meridian Concierge Health L.L.C. is not liable for indirect, incidental, or consequential damages arising from use of the site or from coordination services. This limitation does not affect rights you cannot waive, including claims relating to physician services that remain with the evaluating clinician.

These terms are governed by the laws of the State of Georgia, without regard to conflict-of-law rules, except where a mandatory consumer-protection law of your residence applies.
