Meridian Concierge Health L.L.C. (“Meridian Concierge Health”) is a Wyoming limited liability company with its principal office at 11175 Cicero Drive, Alpharetta, Georgia 30022. We coordinate a private, physician-directed pathway for regenerative evaluation in the United States. We are not a hospital and we do not sell personal information.

When you request a private consultation we collect the contact details, regenerative intake, and lab documents you submit, plus a visit identifier used to keep your upload session together. If you use the private portal we also collect account authentication data (via Amazon Cognito), the records, imaging, and lab documents you upload, check-ins, messages with your coordinator, conversations with Kai (our AI assistant), and coordination records. After your consultation fee is paid, we email you a single-use secure link to a page on the Meridian Concierge website, which opens after you confirm your date of birth, where you can review and sign the Initial Consultation Agreement and the Medical Information Authorization, upload records, imaging, and lab documents, download your signed copy, and revoke the authorization; we keep a record of each link and of each session it opens, including the time and your IP address and browser details. When you sign the Medical Information Authorization, we also email you a copy of it with your date of birth, street address, and telephone number masked; the complete signed copy stays on that secure page, or in the portal if you signed it there. When you accept or sign a document we record the exact text and its version, your typed name, the time, and your IP address and browser details. If a Meridian sales representative referred you, we also receive from them, with your permission, your name, email address, phone number if given, and a short note on how they know you; representatives are told not to include health information. Payment card details, when you pay a fee, are collected by Stripe — Meridian Concierge does not store card or bank account numbers.

We use this information to respond to your inquiry, coordinate a private consultation, operate the private portal and the secure website pages for your authorization and records, process fees, keep a record of the documents you accept, and meet legal or security obligations. Lab documents you upload, through the portal or the secure website pages, are read by automated services inside our Amazon Web Services account (Amazon Textract and Amazon Bedrock) to prepare a coordination summary for your concierge team, which may be sent to our team by email. That summary is not a diagnosis or medical advice. Completing an inquiry is coordination only. It does not establish a physician-patient relationship and does not determine candidacy.

The details and documents you share are used to respond to your inquiry and coordinate a private consultation, as described in our Privacy and Data Use Notice. They are handled confidentially and are not sold, and records reach the treating practice only under an authorization you sign. Using this website or submitting an inquiry does not create a physician-patient relationship. That relationship is established only after a direct evaluation by a licensed physician.

We share your records with the treating physician practice (currently ICCA) only while an authorization you have signed is in effect (the Medical Information Authorization, or a Medical Records Authorization signed before it), and only as that authorization describes. We also use service providers who process information on our instructions, including Amazon Web Services for hosting, automated document reading, and email; Stripe for payments; and Calendly, which receives your name and email when you book a call. Medical judgment, clinical consent, treatment, and the official medical record remain with the treating practice. If a Meridian sales representative referred you, with your permission, that representative sees only the name and contact details they entered and whether your inquiry was received and a consultation reserved — never your health information. We do not sell your information. We do not send diagnoses, lab values, therapy names, or other clinical content by text message.

Meridian Concierge coordinates the client experience, portal, workflow, payments, communications, and follow-up. Medical judgment, clinical consent, treatment, and the official medical record remain with ICCA.

The treating practice is a separate healthcare provider with its own Notice of Privacy Practices, which explains how it uses and protects your health information. When your records are shared with the practice, it posts that notice in your portal documents so you can review it and acknowledge receipt.

Meridian Concierge Health may send optional coordination text messages to a mobile number you provide — for example scheduling, document requests, and appointment reminders. Message frequency varies. Message and data rates may apply. Texts are coordination only and will not include diagnoses, lab results, therapy names, or other clinical content. Consent is not required to request a consultation. We retain inquiry, coordination, and account records for as long as needed to provide the requested coordination and to meet legal, tax, and security requirements, then delete or de-identify them when they are no longer required. Records of the documents you accept or sign are kept as evidence of what you agreed to.

You may email ops@meridianconcierge.health to request access to, correction of, or deletion of personal information we hold, or to withdraw SMS consent. You may revoke a Medical Information Authorization or a Medical Records Authorization at any time from your portal, on the secure website page for your authorization, or by email; revocation does not undo a disclosure already made. We will verify a request before acting. Some records may be retained where the law requires it.

This site and the private consultation pathway are intended for adults. Do not submit information about a child through the public forms.
